Insurance Return to be filled in and returned with fees by your club treasurer by the end of each month
Release Form if a member wishes to change clubs part-way through the calendar year.
Insurance claim form if a member wishes to lodge a claim for insurance

Note: All forms and records must be kept by the club for 5 years under our insurance policy obligations
